OSHA falls into the "We're from the Government, and we're here to help you" category- it's my experience that they normally only appear AFTER an accident or fatality to issue fines.

I complained twice last year about the scaffolds and practices at a construction site; the first time I left a message on the OSHA office's machine and the second time I reached a field agent who transferred me to someone else's answering machine- never heard back from him, either. The construction site supervisor didn't care either. I noted the time & date of both messages and decided that if an accident occurred, I'd be in front of the news cameras with my notes.
